eCard Replacement

For lost eCard

  1. Call any Union Bank / Land Bank branch to inform the loss of the eCard to prevent others from using it.
  2. Execute an affidavit of loss duly notarized by a lawyer.
  3. Go to any Union Bank / Land Bank branch and fill out an eCard replacement form.
  4. Submit the accomplished eCard Plus replacement form with the Affidavit of Loss to the bank personnel.
  5. Pay the corresponding replacement fee.
  6. In a few weeks, you can get your new eCard from the branch where you applied.

For damaged eCard

  1. Present the damaged eCard at any Union Bank / Land Bank Branch.
  2. Fill out an eCard replacement form.
  3. Submit the accomplished eCard  replacement form to the servicing bank.
  4. Pay the corresponding replacement fee.
  5. In a few weeks, you can get your new eCard Plus from the branch where you applied.

Important reminder:

You may still withdraw money from your eCard  through over-the-counter transaction at any Union Bank or Land  Bank Branch. Present valid IDs and your  savings account number.
