eCard Plus Replacement

eCard Plus/Temporary Card/UMID Card/LBP Issued UMID Card Replacement

  • Lost eCard Plus/UMID Card/LBP Issued UMID Card or Forgotten/Lost Personal Identification Number (PIN)
  • Damaged eCard Plus/UMID/LBP Issued UMID Card
  • Transfer of servicing branch from UBP to LBP
  • Expired eCard/UMID Card for Pensioner based abroad

For lost eCard Plus/UMID Card/LBP Issued UMID Card or Forgotten/Lost PIN

  1. Call or send email immediately your Servicing Branch to inform them that your eCard Plus/UMID issued card is missing so they can block your account to prevent others from using it.
  2. Execute an affidavit or loss duly notarized by a lawyer
  3. Go to your servicing branch and fill up an eCard/UMID Card replacement form
  4. Submit to your servicing branch personnel the accomplished eCard Plus/UMID Card replacement form with the affidavit of loss
  5. You will receive a text or email when you can get your new eCard Plus from your servicing branch where you applied.

For damaged eCard Plus/UMID Card/ LBP Issued UMID Card:

  1. Present your damaged eCard Plus/UMID Card at your servicing branch
  2. Fill up an ecard Plus/UMID Card replacement form
  3. Submit to your servicing bank personnel the accomplished ecard Plus/UMID Card replacement form
  4. Pay the corresponding replacement fee
  5. You will receive a text or email when you can get your new eCard Plus from your servicing branch where you applied

For transfer of servicing branch from UBP to LBP

  1. Present your UBP-issued UMID card to the LBP branch, in lieu of the LBP-issued UMID card that will be issued to you
  2. Fill up the enrolment form and the specimen signature card
  3. Submit to LBP bank personnel the accomplished eCard Plus/UMID card enrolment form and specimen signature card
  4. You will receive a text or email when you can get your new eCard Plus from your LBP branch where you applied

For replacement of expired eCard plus/UMID Card for pensioner based abroad:

  1. Send email request for Skype Video Call Conference to pensionglobal@gsis.gov.ph . Be sure you are ready with the preliminary activities (please provide link) for video call conference
  2. Wait for confirmation of schedule from the pensionglobal mailbox.
  3. On the day of your schedule, please be online by logging-on to your Skype username/account and make sure that:
    • Your computer is positioned in a well-lit area
    • Your scanned/photo copy of ecard Plus/UMID and/or 2 Valid IDs or Passport are on hand for identification.
  4. Wait for our call using the gsis.global1 / gsis.global2 / gsis.global3 / gsis.global4 / gsis.global5 / gsis.global6 Skype username/account
  5. During the video call, an interview will be conducted by GSIS Skype Interviewers to determine pensioner’s identity and eligibility.

Upon receiving your eCard Plus/UMID Card:

  1. Get in touch thru email with pensionglobal@gsis.gov.ph
  2. Request for preferred schedule, Manila date and time
  3. Wait for confirmation from pensionglobal@gsis.gov.ph on the preferred schedule
  4. On the day of your schedule, please be online by logging-on to your Skype username/account and make sure that:
    • Your computer is positioned in a well-lit area
    • Your replacement ecard Plus/UMID card and/or scanned/photo copy of ecard Plus/UMID and/or 2 Valid IDs or Passport are on hand for identification
  5. Wait for our call using the gsis.global1 / gsis.global2 / gsis.global3 / gsis.global4 / gsis.global5 / gsis.global6 Skype username/account
  6. During the video call, an interview will be conducted by GSIS Skype Interviewers for the processing of eCard Plus replacement.

  1. Presentation of ecard Plus/UMID and/or two (2) valid IDs are required during the Skype video call conference.
